Instructions to complete application forms for Russian Visa
The form must be completed in blue or black ink.
1. Present citizenship: This is the country you hold a passport from. Do not list your ethic background. For example, if your were born in Italy or are of Italian decent but are now traveling on a US passport, you put “USA.” If you previously held USSR or Russian citizenship, explain when and why you no longer hold that citizenship.
2. Last name: Enter your last name as designated in your passport. If you name has been amended in your passport, enter the last name as it reads in the amendment.
3. First and middle names: Enter your first and then middle names as entered in your passport. If you name has been amended in your passport, enter the names as they read in the amendment.
4. Other names ever used: Enter any previous names you may have had or used. This includes maiden names.
5. Place of birth: Enter the city, state (if applicable) and country where you were born. If born in Russia or The Soviet Union, enter when and to what country you immigrated.
6. Date of birth: Enter 2 digits for the day, then month, and last the year you were born: DD/MM/YY.
7. Sex: Check “M” for male and “F” for female.
8. Purpose of visit: Check the appropriate box. If traveling as a tourist, and applying with visa support from a hotel, tour/cruise operator, check “tourism.” If traveling on business and have corresponding authorization from Russian Foreign Ministry, check “business.” If going to stay with family or friends, and have the approval from the local O.V.I.R. office, check private. If other, check appropriate box, and explain if necessary.
9. Russian institution or organization to be visited: If you checked “tourism” in question 8, enter “not applicable.” If you checked anything other than “tourism” on 8, enter the name, address, phone and FAX number of your contact in Russia.
10. Itinerary: List the cities you will be visiting in Russia.
11. Date of entry: Russian visas are date specific. Enter the 2 digits for the day, month and year of the date you expect to enter Russia . Regardless of the date entered here, visas are issued in accordance with dates on your authorization .
12. Date of departure: Enter the 2 digits for the day, month and year of the date you expect to exit Russia . Again, regardless of the date entered here, visas are issued in accordance with dates on your authorization .
13. Number of entries: Enter the number of times you expect to enter Russia. As with validity dates, number of entries is based upon visa support, not number of entries requested here.
14. Passport no: Enter your passport number from the information page of your passport. Issued by: Enter the name of the authority issuing your passport, for example, US Passport Service. Or if passport was issued abroad, enter the issuing post, such as “US Embassy in Rome.” Date of issue: Enter the 2digit day, month and year of the date you passport was issued. Valid until: Enter the 2-digit day, month and year of the date your passport expires. All this information is found the front of your passport on the page with the photo.
15. Type of passport: Mark “tourist” unless your passport states otherwise, even if you checked something other than “tourism” for question 8.
16. Name and reference number of the tourist group: If traveling on a tour or cruise, enter the name and number of your program. If no on a tour or cruise, enter “Not applicable.”
17. Do you have a medical insurance policy valid in Russia? Enter “yes” and the name of your insurance provider.
18. Who will pay for your trip to Russia? Enter “myself” if applying for a tourism or private visa, per question 8. Otherwise, enter the name of the US Company paying for your travel.
19. Marital status: Check the appropriate box.
20. Spouse's full name: Enter the name of your spouse, even if divorced or separated. Men should enter their wife's maiden name. If none, enter “not applicable.”
21. Spouse's date of birth: Enter the 2 digits day, month and year your spouse from question 20 was born. If none on question 20, leave blank
22. Spouse's place of birth: Enter the city, state (if applicable) and country of where spouse from question 20 was born. If none for question 20, enter “not applicable.”
23. Your father's full name: Enter your father's first and last name.
24. Your mother's full name: Enter your mother's first and last (maiden) name.
25. Have you ever been issued a Russian visa: Check appropriate box, “yes” or “no.” If yes, enter 2 digit day, month and year for when visa was issued. Also, state the city and country where it was issued. Try to answer to the best or your ability. If you are not exactly sure of the date, that is fine. Just put the approximate year.
26. Has your passport ever been lost or stolen? Check appropriate box, “yes” or “no.”
27. List all countries you have visited in the last ten years and indicate the year of visit: It is only necessary to list the counties evident in your current passport. Try to do the best you can with this, it is not that important to be exact with this, especially regarding the dates. If you have too many to list here, attach an addition sheet to the back of the form.
28. List all countries which have ever issued you a passport: List any country that has issued you a passport besides the one you are currently submitting for a visa.
29. List you last 2 places of work, excluding the current one: List the requested information for your prior 2 employers. This needs to be answered, even if you are currently retired or the organization is now defunct. Again, try to answer the information as best you can. If you do not have 2 previous employers, enter “not applicable” as necessary.
30. List all educational institutions you have ever attended, except high schools: List the name of the last one or two, if applicable, institutions of higher learning you have attended. Also list your area of study. If you did not major in anything specific, enter “General Studies.” Just estimate the dates and enter the city and state for address. If you did not study beyond high school, enter not applicable.
31. List all professional, civil and charity organizations which you are (were) a member of or contribute(d) or cooperate(d) with: List, if any, you choose to share. It is acceptable to just answer “none.”
32. Do you have any specialized skills, training or experience related to fire-arms and explosives or to nuclear, biological or chemical activities? Answer accordingly. If yes, please briefly explain.
33. Have you ever performed a military service? Answer “yes” or “no.” If yes, the country where you served, branch of service, military occupation and the years served.
34. Have you ever been involved in an armed conflict, either as a member of the military service or a victim? Answer “yes” or “no.” If yes, please give details.
35. Is any of your relatives mentioned below stay in Russia now? Enter “no” as needed. If yes to any: your spouse, parent, sibling, fiancee or child, answer accordingly and provide requested details regarding their status in Russia.
36. This question has 7 parts. While it is important to complete the application honestly, understand that answering “yes” to any of the following questions will result in significant processing delays:
- Have you ever been arrested or convicted on any offence? Answer “yes” or “no.” If yes, provide requested details.
- Have you ever been sick with a communicable disease of risk for the public or suffered a dangerous physical or mental disorder? Have you ever been a drug addict? Answer “yes” or “no.” If yes, provide requested details.
- Have you ever been refused a Russian Visa? Answer “yes” or “no.” If yes, provide requested details.
- Has your Russian visa ever been cancelled? Answer “yes” or “no.” If yes, provide requested details. A previous visa expiring is not applicable.
- Have you ever tried to obtain or assisted others to obtain a Russian visa or enter Russia by providing misleading or false information? Answer “yes” or “no.”
- Have you ever over stayed your Russian visa or stayed unlawfully in Russia? Answer “yes” or “no.”
- Have you ever been deported form Russia? Answer “yes” or “no.” If yes, provide requested details.
37. Your permanent address, phone, fax number and email:
38. Place of work or study, present position: Provide the requested information. You must put your complete work/school street address, with city, state and zip code. Also, list current position. If a student, reply as such. If unemployed or retired, state which is the case.
39. Name, address and phone number of person ore hotel in Russia that you plan to stay with: Regardless if traveling for business for tourism, list the name of the hotel and the respective cities you plan you plan to stay at in Russia. If on a cruise, put the name of the cruise ship. If applying for a private visa, answer with specifics for your sponsor in Russia.
40. Has anyone assisted you in completing this application form? Answer “yes” or “no.” If yes, proceed to 41.
41. Application completed by: Enter specifics for person that assisted you.
You must date and sign your application.
Incomplete applications or applications that are not signed,
will not be accepted by the Russian Authorities.
